Care For All
We welcome Medicaid (BadgerCare) and most private insurance. We offer a sliding fee scale based on income and family size that may lower or remove your cost. No one is turned away due to inability to pay.

Dial 9-1-1, if you are in an emergency. If you have an issue outside of our regular hours, please contact your local emergency room or urgent care provider. If you are a patient of NorthLakes and are in the hospital, please tell the staff that NorthLakes is your primary care provider so we can provide the best continuity of care.
