NorthLakes announces plans to consolidate and expand its healthcare services in Rice Lake at a new, integrated facility while relocating its Birchwood clinic operations to the same location.
After opening its doors in January 2025, the 1051 West Avenue Rice Lake facility serves behavioral health patients. By June, it will include dental care for all ages, pediatric occupational, speech, and physical therapies, as well as patient supports. Together, these many service lines mark a significant expansion of healthcare services for the community.
The move combines services provided at multiple locations, including the Birchwood Clinic (108 S Main St), Rice Lake Clinic (1035 N Main St), and the new West Avenue location. This strategic relocation will establish one comprehensive healthcare hub to offer a broader array of integrated services under one roof.

The new integrated facility will offer:
- Behavioral Health services

- Patient Supports
Patients from the Birchwood and Rice Lake locations will continue seeing their same healthcare providers in their respective clinics until early June, when all services will move to the new Rice Lake location.
Rice Lake and Birchwood patients with appointments scheduled after June 1, 2025, will have their appointments automatically transferred to the new facility. The clinic is prepared to work with patients concerned about transportation or access to the new location.
NorthLakes will maintain its commitment to accessible healthcare by accepting Medicaid/BadgerCare and offering a Sliding Fee Scale for uninsured or underinsured patients.
