NorthLakes Community Clinic is more than just a newcomer to Eau Claire County; it is a committed, long-term neighbor. Following the closures of Prevea Health and Hospital Sisters Health System, the clinic responded to care gaps and improved access for underserved populations. Now, it has established a permanent fund with the Eau Claire Foundation. The NorthLakes Community Clinic Fund will provide seed funding to explore evidence-based interventions through pilot programs and sustain patient support services.
“Our fund aims to connect patients with clinical services, resources, and support. It will provide funding to expand existing patient programs and new initiatives focusing on integrated care, including the social drivers of health that contribute to well-being,” said Ana Tochterman, NorthLakes Community Clinic CEO.
The new program endowment will provide permanent funding for NorthLakes to continue patient support services and explore new programs to remove barriers to wellness. These support programs, including nutrition and literacy initiatives, address non-clinical factors influencing health outcomes. About 86% of NorthLakes Community Clinic – Eau Claire patients receive Medicaid or qualify for the sliding fee scale, which assists patients up to 200% of the federal poverty level.
“We are proud to work with NorthLakes Community Clinic in establishing their NorthLakes Community Clinic Fund, ensuring they have the resources to support underserved populations in Eau Claire County—now and for generations to come,” said Wes Escondo, president of the Eau Claire Community Foundation.
NorthLakes is actively fundraising for this endowment at the Eau Claire Community Foundation. If you believe in our mission and want to make a difference in the lives of underserved populations in Eau Claire County in perpetuity, please consider donating to our NorthLakes Community Clinic Fund.
