NorthLakes understands that it can be hard to get the care you need, so we bring the care to you. To help reduce barriers to wellness, we bring care to schools, nursing homes, childcare centers, and WIC county offices every year.
School-based Behavioral Health
Our school-based behavioral health program ensures that students receive the mental health care they need at a time that is most convenient for them, based on their course schedule.
Behavioral health can help with:
- improved access to consistent services and treatment
- ability to remain productive in academics and improve school performance
- immediate opportunity to apply newly learned skills
- reduced travel time and cost to and from appointments
- improved ability for parents/guardians to remain at work
- diminished missed appointments
Refer a Student
Call 888.834.4551 to connect to your closest clinic or speak to a guidance counselor at the school to request the service. Fill out the referral form and mail to your nearest clinic.
Dental Outreach
Our dental outreach program provides treatment to students and seniors by licensed dental professionals. Treatment includes oral health assessments, fluoride varnish applications, dental sealants, oral health education, and dental supplies.
NorthLakes has the most extensive Seal-A-Smile outreach in the state of Wisconsin, operating across the northern counties and bringing preventative dental care right to where people are, helping to reduce barriers to wellness.
Community Dental
Our dental outreach program operates within schools, childcare centers, WIC county offices, and nursing homes, providing easier access to dental care.
- Every child is eligible for the program, regardless of their insurance.
- A report will be sent home after each visit, listing the services provided.
- If decay is suspected, a referral will be sent home with information stating the findings.

Care For All
We welcome Medicaid (BadgerCare) and most private insurance. We offer a sliding fee scale based on income and family size that may lower or remove your cost. No one is turned away due to inability to pay.
